CONSENT STATEMENT

I (the client) agree to allow my dog(s) to be groomed by Theo’s as required during my appointment. I understand that grooming may include bathing, clipping, styling, coat treatments, and a basic health check.

I (the client) confirm that my dog is fit and healthy, that all vaccinations are up to date, and that grooming can be carried out safely. Any existing concerns about my dog’s health or behaviour will be discussed prior to the appointment.

I (the client) understand that Theo’s cannot be held liable for skin irritation, abrasions, patchiness, or hair loss that may occur due to pre-existing skin conditions, de-matting, thinning, hand-stripping, shaving, or any undisclosed medical or behavioural issues. If my dog bites or attempts to bite any person or animal, I agree that a muzzle or other safety measures may be used at the groomer’s discretion. If the groomer determines that work cannot be safely completed, the session may be stopped, and I agree to pay for the work completed up to that point.

If my dog is found to have parasites, I understand that grooming will stop immediately and my dog will be returned to me for appropriate treatment.

If any health concerns arise during the appointment, I will be informed upon collection. In the event that urgent veterinary care is needed, I give consent for my dog to be transported to my own vet, or if unavailable, the nearest available vet. If the need for treatment is the result of groomer error, costs will be covered by the groomer’s insurance.

Matted Coats
If my dog’s coat is matted, I understand I have the following options:

1. I may take my dog home to brush out the matting and return at a later date.
    
2. I may request de-matting by Theo’s, understanding this may incur additional charges and will only be carried out if it does not cause excessive stress to my dog.
    
3. I may request a full shave-down to remove the matting. Additional fees may apply.
